Lifestyle Change Program to Prevent Type 2 Diabetes
Type 2 diabetes is a serious condition that can lead to health issues such as heart attack, stroke, blindness, kidney failure, and lower limb amputation.
Having prediabetes means your blood glucose (sugar) level is higher than normal, but not high enough to be diagnosed as diabetes. Without taking action such as losing weight, increasing physical activity and eating healthier, many people with prediabetes can develop type 2 diabetes within five years.
But pre-diabetes CAN BE reversed and we can help you make those important lifestyle changes.
It’s Time to Take Charge of Your Health
You are more at risk to develop type 2 diabetes if you are:
- Overweight
- If you are physically active less than three times a week.
- If you have an immediate family member with the disease, among other risk factors.
We offer the Diabetes Prevention Program to eligible participants (see eligibility requirements and a prescreening test below) to help guide you through lifestyle changes you need to lose weight, have a better relationship with food, be more physically active, and manage stress.
Throughout the program, a trained lifestyle coach will guide you and encourage you as you make these important changes. You will also have the support of other participants who have the same goals as you. The curriculum we use is CDC-approved, so you can feel confident in what you are learning.
This is a year-long program that meets once a week for sixteen weeks, then once every other week for two months, and then once a month for the last six months. It is a commitment worth making to yourself and your health so you can enjoy the rest of your years with more energy, less stress, and fewer medical complications.
The program offers:
- A CDC-approved curriculum
- The skills you need to lose weight, be more physically active, and manage stress
- A trained lifestyle coach to guide and encourage you
- Support from other participants with the same goals as you
- Follow-up meetings to help you maintain a healthy lifestyle
Program goals:
- Lose 5-7% of your body weight
- Getting at least 150 minutes of physical activity per week
- Lowering your A1C

- Be diagnosed with prediabetes by your physician
- OR score a 5 or greater on the Risk Self-Assessment

The 30-minute presentation will provide a general overview of the program and what you can expect as a participant.

Medicare Part B patients may have to meet additional qualifications including a blood test verifying a prediabetes diagnosis.
