St. Joseph’s Health uses a state-of-the-art electronic medical record system. This creates a more secure environment for health information in addition to immediate access for physicians to review the patient's medical record.
How do I request copies of my medical records?
Click on the button below to complete the online request form or return the downloaded/completed form via the fax number, or to the mailing address listed. Please note: a fee may apply when requesting a copy of your records.
