What to Bring
What information should I bring to my appointment/admission?
- All current health insurance cards
- All prescription insurance cards
- Photo ID
- Prescription/order for your test (if your doctor has given you a copy)
- Health Care Proxy/Living Will/DNR or power of attorney (if you have one established)
- Previous X-rays (if applicable)
- Pre-registration confirmation page (if you completed online registration form)
- List of your medications (see following details)
Bring a Medication List
Keep a list of all your medications. This list should include:
- Prescription medications.
- Medications such as inhalers, nitroglycerine, pain medications, eye drops, insulin and aspirin that you use as needed.
- Over the counter medications or herbal products.
- Your allergies and what symptoms you have
This list is important for your safety. Bring this list (pdf) whenever you visit your doctor, enter the hospital or have any medical tests. Whenever you come to our hospital we will be asking for this list. It helps us determine if there is any reason to change your medicines while you are here. It also helps you remember all your medications so we have an accurate list.
